Documentation Requests for Service Animals & FMLA
At Bridging The Gap Psychological Services, LLC, we are committed to providing ethical, clinically sound documentation that reflects an accurate assessment of each client’s mental health needs. To ensure that all letters and forms are supported by appropriate clinical evaluation, the following policies apply:
Service Animal Documentation
Clients requesting documentation in support of a service animal or emotional support animal must:
Complete a minimum of three (3) clinical sessions to allow for proper assessment and clinical determination.
Pay a $150 documentation fee, which covers the time required for record review, clinical evaluation, and preparation of the letter.
Documentation will only be provided when clinically appropriate and supported by assessment findings.
FMLA Documentation (If Not Covered by Insurance)
Clients requesting completion of FMLA paperwork must:
Pay a $250 documentation fee if the service is not covered by insurance.
Attend weekly therapy sessions at a discounted self-pay rate for the duration of their approved leave to ensure continuity of care and clinical support.
FMLA documentation will be completed based on medical necessity and consistent treatment participation.
